MISSION

In carrying out our mission, the School-Based Wellness Centers will:

  1. Become a recognized center for quality care involving the most common occurring mental and physical illness of school-aged youth.
  2. Develop relationships with community members to create a network of support systems to enhance the services provided by the centers.
  3. Develop new relationships with community members to include: patients/students, physicians, employers, providers, underserved, uninsured and third party payers.
  4. To be the leader in quality health care and educational outcomes as judged by our customers.
  5. To be the health care provider of choice by our customers.
  6. Cooperate with other providers to decrease cost and promote health and wellness.

It is the vision of the Pueblo School-Based Wellness Centers to provide quality services and improve access for school-aged youth in order to improve their health and wellness.

  • All services for the prevention of pregnancy such as contraception and condoms are referred to a primary care provider in the community.

ENROLLMENT

The students must have a consent signed by the parent(s) or guardian before any treatment is provided. No student will be permitted to use the centers without the signed consent form on file. All visits are confidential. Parents will be notified of visits when applicable. This excludes the confidential visit mandated by Colorado State law. The centers highly encourage parental involvement. The Centers an leave open door policy and welcome all school-aged youth.
